Effectiveness of Case Management

Two reasons why case management is effective as a tool;
· First, continuity of care, a principal goal of case management, is to keep clients engaged and moving forward.
· Second, clients are more likely to succeed when problems are addressed holistically and concurrently.
Case management focuses on the whole individual and stresses comprehensive assessment, goal planning, and service coordination to address multiple aspects of a client's life. Comprehensive planning often requires that clients move to different levels of care or systems; case management facilitates such movement.
